Bei der sogenannten Vascularised Composite Allotransplantation (VCA) werden nicht Organe sondern Körperteile wie z.B. ein Arm oder ein Bein transplantiert. Das Transplantat enthält damit neben Haut, Blutgefässen, Muskeln, Nervenfasern und Sehnen auch Knochenmark und Knochenmantel. Bei einer solchen VCA wird das Immunzellen bildende Knochenmark des Spenders mit transplantiert. Nach der Transplantation befinden sich also eigentlich zwei Immunsysteme in einem Organismus.
